FREE NEWSLETTER REGULATIONS

In accordance with Article 8.1 of the Act of 18 July 2002 on Providing Services by Electronic Means (Dz. U. [Journal of Laws] No. 144, item 1204, as amended), the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota located at ul. Szczęśliwicka 36, 02-353  Warsaw, publishes below the regulations regarding the use of the free newsletter:
 
 

FREE NEWSLETTER REGULATIONS

 
§ 1. General provisions 
  1. The Regulations define the scope and conditions for the performance of services provided by electronic means for information purposes by the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota.
  2. The administrator of the personal data obtained as a result of completing the form is the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota. Personal data (i.e. personal data disclosed in the e-mail address) is collected only for the purpose of the implementation of the newsletter service. The data will not be revealed to third parties in accordance with art. 7 item 6 of the Personal Data Protection Act. The user of the newsletter service, to whom the data belongs, has the right to access their data and the possibility of correcting it. Providing the data is voluntary, but without doing it the newsletter service is not possible.
  3. Consent to comply with the regulations when starting the service using the website is tantamount to full acceptance of the terms of the regulations without the need to draw up a separate contract.
  4. The registration form, which is also an order to receive such services and the acceptance of these Regulations, is available at: www.szpzlo-ochota.pl
  5. Before agreeing to use the newsletter services, read these regulations carefully.
  6. A USER within the meaning of the Regulations is a natural person, legal entity or organizational unit without legal personality, using the newsletter service as a result of sending a completed Form in accordance with these regulations.
 
§ 2. Type and scope of services covered by the regulations 
  1. The Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota provides a newsletter service.
  2. The Newsletter is a free service, consisting of informing the Users about the services provided by the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota by periodically sending the content selected by the editors in the form of an electronic letter.
 
§ 3. Terms of providing services 
  1. A computer with Internet access, any web browser and an active and properly configured e-mail service is required to use the service.
  2. In order to order the newsletter, the User fills in the form provided by , the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota on the website at www.szpzlo-ochota.pl. To the e-mail address provided by the User will be sent information containing a link confirming and activating the ordering of the newsletter.
  3. The User bears responsibility for the validity of personal data he/she has indicated in the form.
  4. As part of the newsletter service, the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota will not share the obtained e-mail addresses with third parties, except for cases specified in the Personal Data Protection Act of August 29, 1997 (Journal of Laws No. 133 of 1997, item 883, as amended).
  5. The use of the newsletter service is voluntary and valid for an indefinite term.
  6. The User has the right to delete his/her data from the newsletter service at any time by clicking on the link attached to each newsletter received by the User. In order to update the data, please send the updated data to: sekretariat@szpzlo-ochota.pl.
  7. By subscribing to the newsletter, the customer agrees to be sent information via the newsletter about the services provided by the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota to the User's e-mail.
  8. The electronic address will be used only to send the ordered information. The User undertakes not to send unlawful content (Article 8 paragraph 1 item 3 letter b).
  9. The Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota reserves the right to delete an account whose user has violated these regulations or legal provisions applicable in the Republic of Poland, or takes actions to interfere with the uninterrupted use of the service by other users.
 
§ 4. Complaint proceedings
  1. The Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota makes utmost care to ensure an appropriate level of newsletter service.
  2. The user using the newsletter service has the right to file complaints regarding this service to the following address: sekretaria@szpzlo-ochota.pl.
  3. The complaint referred to above in paragraph 1 should contain a description of the problem occurring with the use of the newsletter service and the User's contact details, enabling the transfer of the response to the complaint.
  4. Answers to the complaint will be provided by SZPZLO Warszawa - Ochota within 14 days after receiving the complaint.
 
§ 5. Final provisions
  1. In matters not covered by these regulations, the relevant provisions of the Civil Code and the Act on electronic services shall apply.
  2. Any disputes will be settled by a common court competent for the seat of the Independent Group of Public Ambulatory Care Institutions Warsaw-Ochota.
  3. The Regulations shall enter into force on the day of its publication.
