The Medical Care Home, run on a day-care basis, was created for people diagnosed with a dementia/syndrome in the mild to moderate phase of the disease, whose health condition does not allow them to be left solely under the care of primary health care and outpatient specialist care, and at the same time does not require 24-hour medical and nursing supervision carried out in an inpatient setting. The patient does not incur any fees for using DOM services.
People who meet all of the following criteria at the time of application may be eligible for DOM:
The decision to admit an applicant to a Medical Care Home is made by the therapeutic team operating in the entity running the Home based on an interview with the potential participant, psychological tests, analysis of medical records, interviews with the family/guardians of potential participants and possible observation of their mental and physical condition.
The aim of the DOM is to support people with dementia through therapeutic and rehabilitation activities and to improve their quality of life, including extending the time of mental and physical activity, improving cognitive functions and functioning in society, as well as meeting health and social needs, developing the ability to function independently and interpersonal skills. DDOM also supports families/caregivers of sick people through education on dementia and the opportunity to participate in consultations with specialists (including: psychiatrist, nurse, psychologist, dietitian, social worker).
The Medical Care Home is run by the Independent Complex of Public Open Healthcare Facilities Warsaw-Ochota, as part of the public task entitled "Medical Care Home Support Program", co-financed from the budget of the Mazowieckie Voivodeship.
