Registration for consultations and visits, including home visits
a) to the primary care physician - Regulations for providing teleconsultations
b) to specialist doctors, dentists, rehabilitation, psychiatric care
The healthcare provider provides healthcare services and specialist services in outpatient healthcare on the day of registration.
If the service cannot be provided on the day of the request, the provider will schedule an alternative date, regardless of the order of admissions based on the waiting list maintained by the provider. Outpatient specialist care cannot be provided later than 7 business days from the date of the request.
- Registration for a doctor's appointment is made during the registration office's opening hours.
- Benefits are provided as soon as possible.
- Registration takes place in person, by phone, or through a third party, in the order in which applications are received:
- for the current day, specifying the time of the visit/advice,
- in advance for a specific date, specifying the date and time of the visit/advice,
- for organizational reasons, home visits are accepted on the same day in the morning,
- Scheduled home visits are booked in consultation with the attending physician and are carried out as soon as possible;
- patients are registered in the order of registration; if the service cannot be provided on the day of registration, the date of its implementation is agreed with the patient at registration and the patient is then placed on the waiting list;
- when the date of the appointment at the specialist clinic is distant, the patient is informed about the possibility of seeking specialist advice at other facilities that have a contract with the National Health Fund;
- In the event of unforeseen circumstances that prevent the service from being provided on the agreed date, the patient is informed of the change of date by telephone.
- Persons entitled to use healthcare services financed from public funds out of turn:

- children up to 1 year of age,
- having the title of a distinguished honorary blood donor,
- having the title of distinguished transplant donor,
- war and military invalids,
- veterans and anti-communist opposition activists or persons repressed for political reasons,
- injured veterans, in the scope of treatment of injuries and diseases acquired while performing tasks outside the country,
- pregnant women,
- patients up to 18 years of age, with a certificate of severe and irreversible impairment or incurable life-threatening disease that arose in the prenatal period of the child's development or during delivery (in accordance with Art. 47 sec. 1a of the Act of 27 August 2004 on health care services financed from public funds (Journal of Laws from 2017, 1938 as amended).

PATIENTS WHO HAVE A DOCUMENT CONFIRMING THE ABOVE-MENTIONED PERMISSIONS ARE EXEMPT

Registration for the waiting list
Waiting list – is a list of people who report to the clinic with a given health problem and who were not provided with health care on the day of registration.
- Patient admissions are carried out in accordance with the order of entries on the above-mentioned list, and acceleration by moving on the list may only be caused by medical reasons.
- Entry onto the waiting list is made on an ongoing basis, on the day the patient registers, during the clinic's opening days and hours.
- Two medical categories of patients are taken into account when registering:
- urgent case – if there is a need to provide urgent services due to the dynamics of the disease process and the possibility of rapid deterioration of health or a significant reduction in the chances of recovery.
- stable case – a patient who is not in a condition requiring urgent medical intervention and has not been classified as an urgent case.
We would like to inform you that in order to receive a given benefit, you can only sign up for the waiting list of one service provider.
The original referral must be submitted by the patient within 14 business days of being placed on the waiting list.
The patient can provide the original referral:
- personally,
- through third parties,
- post.
Failure to submit the original referral within the deadline specified above will result in removal from the waiting list.

If the service cannot be provided on the day of registration, the date of its implementation is agreed with the patient at the registration desk and the patient is then placed on a waiting list;
If the appointment at a specialist clinic is far away, the patient is informed about the possibility of seeking specialist advice at other facilities that have a contract with the National Health Fund;
If unforeseen circumstances arise that prevent the service from being provided on the scheduled date, the patient will be informed of the change by telephone.

Rules for sharing medical records
Medical records are made available to the patient or his/her legal representative, or to a person authorized by the patient under the terms of the Act of 6 November 2008. on patients' rights and the Patient Rights Ombudsman (consolidated text: Journal of Laws from 2017 item 1318 as amended):
- for inspection, including health care databases, at the premises of the entity providing health services
- by preparing extracts, copies or copies thereof for a fee
- by issuing the original against acknowledgment of receipt and subject to return after use, if an authorized body or entity requests the originals of this documentation
The fee for providing medical records is specified in the Organizational Regulations of SZPZLO Warszawa-Ochota.
